Please make sure to bring actual mammogram films, (compact discs will not be accepted) as well as mammogram, ultrasound and pathology reports from the past two years so we may evaluate your medical history.

If possible, please print, complete and bring all patient forms.

Please bring a list of all current medications.

Please bring a copy of your insurance card and identification as well. If your insurance company requires a referral, please obtain it from your primary care physician prior to your visit here. Failure to obtain a referral may result in charges to you.  Co-pays and deductibles are collected at the time of your visit.

If you have new insurance, please call us before your visit so we may verify your new insurance benefits.

Please note that “No Show” or “Cancelled” appointments are subject to a cancellation fee if our office is not notified 24 hours before your scheduled appointment. Please understand that we make every attempt to see our patients within their appointment time, but due to unforeseen circumstances, we may occasionally run late.
